There are two ways to get DA 1380. One way is to search through the Internet. Another way is from your unit. With DA 1380, how do you get started?
This is a scene.
You reported your first stage airborne sports to Fort Eustis one morning in their aviation medicine. Phase 1 will take you through the morning. You finished eating at lunch time.
You will be back in the second phase in the following week. After Phase 2, you will be returned to your unit and provide them with a copy of your airborne physical results. When you do this in your organization, you also prepare and submit a school application.
When you are there, you decide to sign the missing field equipment you need for your upcoming field training. You see your supply sergeant. After you sign the required items, he will inform you that there are other items in the combat supply for you to use.
When you finish all of this, you still have 15 minutes to eat lunch at DFAC.
How do you record this scene on the DA 1380?
If you have a form of DA 1380 that you can fill out, please open it. If you have a blank copy, make another copy and fill it in with a pencil. If you do this manually, use a black pen on the actual copy for a neat effect.
Block 1 from
It is a "date." Since this is related to compensation or things that may affect your pay, you want to use the date format of DFAS. This is the "YYYYMMDD" format.
Block 2 from
Is "From:" Print or enter the address of your organization and your organization.
Block 3 from
It is the "retirement year end date".
Your retirement year end date [RYE] is based on when you originally started the Army Reserve contract. This is based on your continuous contract. For example, your retention contract begins on November 1. Your RYE will begin on October 31 of the following year.
Before April 1, 2010 to October 31, 2014, please say that you are in active service. Your retention contract begins on November 1, 2014, the second day after the last day of the effective contract. Your current RYE will remain on October 31, 2015, not March 31, 2015, as your initial active reserve contract will determine your RYE in a continuous reserve run.

Populate this block with the "YYYYMMDD" format.
Block 4 from
Is the "to" block. This will be the address of the staff who updated your records. For soldiers in the Force Plan Unit [TPU], this is usually your full-time support staff.
These workers [mainly one of the unit administrators] will process your DA 1380.
Type "Commander" all capitalized in this block. Under it, type the address of the organization where the support person is located.
If you use the "N" code, your support staff will handle this for the retirement point. If you use the "P" code, they will process your DA 1380 retirement points and payments. If your support staff is unable to handle the matter in the Prep Center, they will forward this to Camp S1.
If you are a member of the Individual Mobilization Elevator [IMA] or Personal Ready Reserve [IRR], you will use the HRC address to update your records.
Block 5 from
It is your name. Type your name in the block in the order in which the block labels appear.
Block 6 from
It is your score. Enter your salary level. This is the alphanumeric code of your ranking.
Block 7 from
It is your branch. Enter your Soldier's Concentration Area [AOC] code/Major Military Professional Skills [MOS] in this block.
Block 8 from
Leave blank without block 4, unlike the assignment unit.
Block 9 from
Gives you the opportunity to mark the training, responsibilities or guidance you are involved in.
If you select "Other," type a description in the box next to "Other."
Column 9, column A from
, is the date you attended the training. If the day, month, and year are in separate columns, use the DD MM YYYY format in the day, month, and year columns, respectively.
Column 9, column B from
, list the training time you have done.
Column 9, column C from
Is the retirement points you get. There are two different codes that can mark any place where you earn retirement points and pay or earn retirement points.
The "N" code marks non-paid time. You can only get retirement points. The "P" code indicates the time of payment and you can earn retirement points and wages.
One of the engraving codes next to "1" indicates a 4-hour training block; if you attend the funeral honor as a member of the funeral details, it takes 2 hours. "2" indicates two training blocks. You can only get a maximum of "2" per day.
With the above onboard physics scene, you can use "N1". The first phase is completed in the morning and part of the lunch time, about 4 hours. "N1" will give you a retirement point, but no compensation.
In another case, the soldier performs a full day of RST, which will be marked as "P2" that day. "P2" will provide you with two retirement points and pay for two training sessions, or two training days.
Column 9, column D from
, list the nature of the duties you performed, or the training/guidance you received. For onboard physics scenarios, you can put "airborne physics, the first phase, in Fort Eustis, Virginia. Retirement only."
For the RST scenario, the example described may be "Rescheduled Training [RST] to compensate for the missed battle rally on October 25, 2014." The second RST day will proceed to the next line, assuming that the training for the entire day has been completed. The same information. The comment in this line can be: "Rescheduled Training [RST] to compensate for the missed battle rally on October 26, 2014.
This situation assumes that the soldiers did two RST days to make up for the lack of two days of training.
Block 10 from
It is the type/level and position of the police/NCO/official, they observe you, or know that you are performing duties on the DA 1380. The names of the 10 support staff observed were related to Regions 10 and 11. .
Section 11 from
Is the signature of the soldier or qualified official listed in District 10.
Some additional instructions:
1. If you have been assigned to a unit, please submit this form to your organization. If you are assigned to an IRR or IMA, please submit this form to the Human Resources Command [HRC]. Submit it to the office that manages your payroll, records and points.
2. If you are assigned to a unit and you submit this form directly to your Regional Personal Action Center [RPAC] or HRC, you will be overwhelmed. This reduces their ability to perform their assigned tasks.
3. Cover one month with a DA 1380. If your training event overlaps for two months, please submit two DA 1380s, one per month.
3. If you do it yourself, please do not complete the DA 1380 for your Army Correspondence Course/Computer Course.
If your commander authorizes you to enroll in the course and complete the course in writing so that you are paid, then complete the DA 1380 for the course you just completed. Submit the completed certificate and completed DA 1380 to the person who manages your records. Please consult your organization for specific policies.
4. According to the Army Reserve Fund and your unit, you can get paid and complete your own structured development course. Check your organization's policies. If this applies to you and you want to get paid, please bring the certificate and the full DA 1380 to your organization.
5. If your unit enters the completed structured development course in accordance with the AT order, then you can only get paid for the AT order. Use the certificate as a document to complete the course.
