Your co-payment at the time of each visit is the same as HMO. There are no forms to fill out and no deductible. However, there are no gatekeepers with PPOs, so you can find a medical professional without first seeing your primary care physician.

Service point plan

POS programs are more expensive than HMOs and PPOs, but offer the greatest flexibility for three health insurance plans. They provide health care treatment through a network of health providers, but you can also choose to see a doctor.

The doctor's access fee on the network is roughly the same as the cost of HMO and PPO, and you will receive up to 80% of your network access expense reimbursement after you pay the deductible.
